Jobsite: On or about January 11, 2024, the employer did not ensure that two employees digging an excavation approximately 21 feet, 9 inches wide by 38 feet, 8 inches long by 9 feet 6 inches deep were trained to recognize the hazards of working in and around excavations, and how to avoid those hazards.

Protection shall be provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  Jobsite: On or about January 11, 2024, the employer did not ensure that spoil piles were kept at least two feet back from the edge of an excavation.
