29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  a.  At the rebar manufacturing shop, on or about January 17, 2024, on the floor around the RMS Shearline motor, the employer exposed employees to slip, trip, and fire hazards, in that oil was allowed to pool on the floor with a damaged power cord emersed in the oil puddle.

1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.  At the rebar manufacturing shop, on or about January 17, 2024, the employer exposed employees to amputation and crushing injuries from the unexpected release of hazardous energy, in that the employer had not performed periodic reviews at least annually of the written energy control procedures that outlined the scope, rules, and steps for the control of hazardous energy for employees required to perform maintenance and service on RMS Hydraulic Shearline, Model 611, S/N: 306,  the RMS Rebar Bender Machine, Model 14, S/N: 14-M-401, and the RMS Radial Bender, S/N: 229 industrial machine.

At the rebar manufacturing shop, on or about January 17, 2024, the employer exposed employees to an amputation hazard, in that the rebar shear unit of the RMS Shearline machine was missing the output protective guard.

At the rebar manufacturing shop, on or about January 17, 2024, the employer exposed employees to caught-in hazards from ingoing nip point and rotating parts, in that the infeed and output conveyors of the RMS Shearline machine were missing protective guards for the belt and pulleys transmitting power to drive the conveyor rollers.

At the rebar manufacturing shop, on or about January 17, 2024, the employer exposed employees to an electric shock hazard, in that, the RMS Shear machine had two open junction boxes missing protective covers, exposing live conductor wires.
