29 CFR 1910.333(a)(1): "Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them: On or about 01/10/2024 at the job work site located at 1760 County Road 344, Caruthersville, MO., employees were exposed to an arc flash hazard. The exposed employee(s) were troubleshooting fuses within an electrical panel rated at 480 volts / 400 amperage and live parts were not deenergized ahead of time. Lockout-tagout procedures were not in place at the time of the incident, nor were employees trained upon or qualified to be working inside of the panel.

29 CFR 1910.333(c)(2): "Work on energized equipment." The employer did not ensure that only qualified persons may work on electric circuit parts or equipment that have not been deenergized under the procedures of paragraph (b) of this section. Such persons shall be capable of working safely on energized circuits and shall be familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools. On or about 01/10/2024 at the job work site located at 1760 County Road 344, Caruthersville, MO., employees were exposed to an arc flash hazard. The exposed employee(s) were troubleshooting fuses within an electrical panel rated at 480 volts / 400 amperage with a personal 24 voltage meter. Employee(s) were not qualified to be using a voltage meter or any electrical testing equipment to troubleshoot electrical problems at the facility.
