29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:  (a)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that electrical equipment was effectively closed, in that employees were exposed to live electrical circuits associated with a missing knockout on the Wadsworth power panel on the west wall in the Old Railroad Building.  This condition exposed employees to shock and electrocution as well as fire hazards associated with exposed wiring on the electrical box.  (b)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that electrical equipment was effectively closed, in that employees were exposed to live electrical circuits associated with a missing electrical breaker in an electrical power panel on the north wall of the Warehouse Office area.  This condition exposed employees to shock and electrocution as well as fire hazards associated with exposed wiring on the electrical box.  (c)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that electrical equipment was effectively closed, in that employees were exposed to live electrical circuits associated with a missing conduit line cover plate on the east wall conduit line above the power panel in the Quonset Building.  This condition exposed employees to shock and electrocution as well as fire hazards associated with exposed wiring on the electrical conduit lines.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that the electrical panel on the north wall in the Flooring Garage had sufficient access and working space in that materials was obstructing the panel.  This condition exposed employees to electrical hazards due to the blocked electrical panel.  (b)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that the electrical panels on the north wall in the Warehouse Office area had sufficient access and working space in that materials were obstructing the panels.  This condition exposed employees to electrical hazards due to the blocked electrical panels.  (c)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that the electrical panel on the east wall in the Quonset Building had sufficient access and working space in that materials was obstructing the panel.  This condition exposed employees to electrical hazards due to the blocked electrical panel.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts, or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  (a)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that electrical equipment was effectively guarded, in that employees were exposed to live electrical wiring associated with a missing wire connector or an approved cover on the north wall of the Warehouse Office area.  This condition exposed employees to shock and electrocution as well as fire hazards associated with exposed live electrical wiring less than seven feet from the floor.

This condition exposed employees to  trip and egress hazards.

This condition exposed employees to struck-by hazards associated with inoperable powered industrial trucks.  (b)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that a Mitsubishi powered industrial truck, Model Number: FB20NT, Serial Number: EFB1450544 was being examined at least daily prior to use.  This condition exposed employees to struck-by hazards associated with inoperable powered industrial trucks.  (c)Larry's Lumber, Inc. at 183 North Maple Street Northeast, Watertown, SD 57201:  On or about and at times prior to January 17, 2024, the employer did not ensure that a Mitsubishi powered industrial truck, Model Number: FG33N, Serial Number: AF13G50708 was being examined at least daily prior to use.  This condition exposed employees to struck-by hazards associated with inoperable powered industrial trucks.
