29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: On or about January 10, 2024, and at times prior thereto, employees operating forklifts after being involved in forklift incidents did not receive refresher training as demonstrated by the successful completion of the training and evaluation specified in 29 CFR 1910.178(l). On January 9, 2024 an employee was involved in a forklift incident and did not receive retraining and evaluation prior to being allowed to continue to operate forklifts.
