29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Warren Transport Inc: On or about January 18, 2024 and at times prior, employees were exposed to struck by hazards while operating the John Deere 322D (IT0322DMEAO188705, Unit # 1005), with an unapproved hole in the forklift fork used to mount a trailer ball to move trailers in and out of the maintenance shop.
