29 CFR 1926.601(b)(4)(i): The employer used motor vehicle equipment with an obstructed view to the rear, and did not have a reverse signal alarm audible above the surrounding noise level: Near 5718 Higdon Road, San Antonio, TX: On or about January 18, 2024 and at times prior thereto, the employees working at ground level were exposed to struck-by hazards due to the operator of the skid steer transporting soil without the backup alarm working.
