29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: a) On or about, January 19, 2024, the employer did not ensure that employees were trained in the skills required for safe application, usage, and removal of energy control devices. Employees performing machine cleaning activities were not trained to lockout the equipment to protect against crushing hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. a) On or about, January 19, 2024, the employer did not ensure that each authorized employee engaged in service and maintenance activities on, but not limited to, the tote dumper affixed a personal lock to either an energy isolating device or a group lockout device. As a result, employees in the cereal room were exposed to crushing hazards when cleaning the equipment. b) On or about, January 19, 2024, the employer did not ensure that each authorized employee engaged in service and maintenance activities on, but not limited to, the cereal hopper affixed a personal lock to either an energy isolating device or a group lockout device. As a result, employees in the cereal room were exposed to caught-in and crushing hazards when cleaning the equipment. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
