At the mulching yard located at 9333 Northwest 13th Street, Gainesville Florida, 32097: On January 19, 2024, and at times prior, the employer exposed employees to struck-by and crushed-by hazards, in that they did not have a proper lock out and tag out program for employees working on the Mobark Horizontal Grinder, specifically when changing the mill inserts.
