29 CFR 1910.217(b)(4)(i):Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals: On or about January 16, 20244, provisions were not made to prevent unintended operation (actuation) of a foot treadle of the Wysong Slip Roll Press, Model B-48, Serial #PR14-318 due to falling or moving objects or by accidently stepping on the treadle, thereby exposing employees to amputation type injuries.
