29 CFR  1910.305(b)(1): Openings through which conductors enter boxes, cabinets, or fittings were not effectively closed.   On January 19, 2024, and times prior thereto, the employer had knockouts open located on electrical power boxes and failed to have them effectively closed exposing employees to electrical hazards.  a) Power box that energize the transformer missing knockout plugs, b) Power box that energize the forklift charging station missing knockout plugs.
