29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface on a ladder that was used to gain access, nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  146 Civic Center Drive - The ladder rails on a portable ladder that was used for access to an upper landing surface did not extend 3' above the upper landing surface, nor was it secured at its top to a rigid support.
