29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. a) Building 36, Second floor deck - On or about January 14, 2024 and at times prior thereto, employees were using an 18' extension ladder that had a number of dented, bent, damaged parts including rails, feet, and rungs.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a) Work site - On or about January 18, 2024 and at times prior thereto, the employer did not report an injury requiring hospitalization within 24 hours. The incident occurred on January 14, 2024 at 1053am and was not reported to OSHA until January 16, 2024 at 2:35am.
