29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: (a) On or before January 22, 2024, the employer did not ensure that employees working in the riveting and production area were protected from injury in that two large openings in the metal floor grates were present and not guarded or protected in the walkway exposing employees to slip, trip and fall hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On January 22, 2024, and at times prior, employees were exposed to caught in hazards from rotating parts and struck by hazards from flying chips while utilizing an unguarded Sharp Industries Drill Press (Model #LIIV, Serial # 30528313).

29 CFR 1910.252(b)(2)(iii): Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles. (a )On or before January 22, 2024, the employer did not ensure employees were protected from potential arch flash in that an employee was welding in one location without shields in the livestock production area that had limited employee traffic.
