29 CFR 1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Envases Ohio, LLC- Fremont, Ohio: On or about January 17th, 2024 the employer did not ensure that employees working in the Fstyle lids line otherwise known as the rework station were provided with necessary PPE for skin and body protection when working with the chemical VEHF-3101-E also referred to as Lacquer-VEHF-3101-E in that employees are exposed to serious skin irritation and chemical burns from the following chemicals contained in this product toluene, ethylbenzene, xylene, and 2-pentanone, 4-methyl 108-10-1.

29 CFR 1910.132(d)(1):The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: a) Envases Ohio, LLC- Fremont, Ohio : On or about January 17th, 2024 the employer failed to perform a PPE hazard assessment for the Fstyle lids line otherwise known as the rework station for employees handling the chemical VEHF-3101-E also known as VEHF-3101-E lacquer to ensure employees are protected against hazards associated with the chemical. Chemical VEHF-3101-E contains the following chemicals toluene, ethylbenzene, xylene, and 2-pentanone, 4-methyl 108-10-1. Potential hazards associated with employee exposure to these chemical include but not limited to chemical burns, skin irritation, eye irritation, respiratory irritation, drowsiness, dizziness, cancer, and in severe instances fatal.
