29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Jobsite located at 3151 Market St, Philadelphia, PA.: On or about January 18, 2024 and times prior, employees used an open flamed portable propane heater to warm themselves. The employer did not ensure that each employee using or working around the portable heater was instructed to recognize and avoid burn and fire hazards associated with the use of open-flamed heaters. Condition was noted on or about January 18, 2024.

29 CFR 1926.252(c):All scrap lumber, waste material, and rubbish was not removed from the immediate work area as the work progressed. a) Jobsite located at 3151 Market St, Philadelphia, PA.: On or about January 18, 2024, the employer did not ensure waste materials such as, but not limited to cardboard boxes, wood crate boxes, etc. were removed from the immediate work area of the 13th floor where employees conducted activities involving curing fireproofing insulation. Condition was noted on or about January 18, 2024.
