29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about January 18, 2024, and times prior thereto employees conducted mesh reinforcement wire installation in stairway treads and landings in preparation for concrete pouring operations. The employees did not wear protective helmets to protect from impact to the head or from falling or flying objects or from electrical shock and burns.

29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems: On or about January 18, 2024, and times prior thereto, employees conducted mesh reinforcement wire installation in stairway treads and landings in preparation for concrete pouring above 6 feet along an unprotected edge, without a fall protection system in place. The employees were exposed to fall hazards.
