29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: At the jobsite located at 150 N. Ridge Rd. in Heston, Kansas, employees engaged in steel erection activities were exposed to fall hazards in that the staircase leading to the upper level was not equipped with handrails.

29 CFR 1926.1428(a)(3): The employer did not make the documentation for whichever option is used available at the site while the signal person is employed by the employer. The documentation did not specify each type of signaling (e.g. hand signals, radio signals, etc.) for which the signal person meets the requirements of paragraph (c) of this section. At the jobsite located at 150 N. Ridge Rd. in Hesston, Kansas, employees engaged in crane operations were exposed to struck-by hazards in that the employer did not provide documentation at the site to show that signal person had been evaluated and had met the qualification requirements.
