29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) On or about and at times prior to January 23, 2023, the employer did not ensure two employees moving a JLG 340AJ articulating boom lift with the basket elevated 10 feet were using a personal fall arrest system with a lanyard attached to the boom or basket. This hazard exposed two employees to a fall hazard of approximately 10 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of aerial lift being used and to understand the procedures to control or minimize those hazards. (a) On or about and at times prior to January 23, 2023, the employer did not ensure two employees moving a JLG 340AJ articulating boom lift with the basket elevated approximately 10 feet were provided with adequate training. This condition exposed the employees to a fall hazard of approximately 10 feet due to not recognizing the hazards of working in the basket of the lift without the use of a personal fall arrest system with a lanyard attached to the basket or boom.
