29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Majestic Metals, LLC:  On or about January 22, 2024, and at times prior, the employer did not ensure machine specific energy control procedures were developed, documented and utilized to control potentially hazardous energy involving machines with multiple sources of energy when employees were engaged in activities covered by this section, including but not limited to the following machines or equipment.  This condition exposed employees to struck-by, laceration, and amputation hazards:  (1)	Amada HG 1003 ATC  (2)	Amada EMK 3612 M2 (3)	Amada EMK 3510 M2 (4)	Amada Pega 357 (5)  Amada Vipros 358 King (6)  Amada HG 2024 (7)  Amada M-2045 (8)  Amada Fab 35  Abatement Note:  Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  (1)	A specific statement of the intended use of the procedure; (2)	Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (3)	Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (4)	Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.212(a)(1): The employer did not ensure one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, in-going nip points, rotating parts, flying chips and sparks:  (a) Majestic Metals, LLC:  On or about January 22, 2024, and at times prior, the employer did not ensure that the point of operation on the Amada EM K 3510 MII was properly guarded to prevent employees from having any part of their body in the danger zone during specific machine operations. The punch turret and floating table moved according to the punches involving the sheet metal and was not guarded effectively to protect the operator. This condition exposed employees to struck-by hazards.   (b) Majestic Metals, LLC:  On or about January 22, 2024, and at times prior, the employer did not ensure that the point of operation on the Amada EM K 3612 MII was properly guarded to prevent employees from having any part of their body in the danger zone during specific machine operations. The punch turret and floating table moved according to the punches involving the sheet metal and was not guarded effectively to protect the operator. This condition exposed employees to struck-by hazards.   (c) Majestic Metals, LLC:  On or about January 22, 2024, and at times prior, the employer did not protect employees from laceration and amputation hazards by use of appropriate guarding methods on the FMB Dentauro horizontal bandsaw. The bandsaw was equipped with an extensible guard that had approximately three inches of the unused blade exposed while cutting smaller diameter stock. This condition exposed employees to lacerations and amputation hazards.

29 CFR 1910.215(a)(2): The safety guard shall cover the spindle end, nut, and flange projections. The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard:  (a) Majestic Metals, LLC:  On or about January 22, 2024, and at times prior, the employer did not ensure employees were protected from struck-by hazards involving a pedestal grinder which was not provided with safety guards covering the spindle end and nut flange projections.  This condition exposed employees to abrasions and lacerations.

29 CFR 1910.1026(l)(1)(iii): Employers shall include chromium (VI) in the hazard communication program established to comply with the HCS (� 1910.1200). Employers shall ensure that each employee has access to labels on containers of chromium (VI) and to safety data sheets, and is trained in accordance with the requirements of HCS and paragraph (l)(2) of this section:  (a)	Majestic Metals, LLC:  On or about January 22, 2024, and at times prior, employees were exposed to chemical hazards while working with chemicals such as hexavalent chromium. The employer did not ensure that hexavalent chromium was included in the Hazard Communication Program (HAZCOM).  This condition exposed employees to hazards associated with hexavalent chromium.

29 CFR 1910.303(g)(1)(i): Working space for electrical equipment likely to require examination, adjustment, servicing, or maintenance while energized did not comply with the following dimensions (3 feet), except as required or permitted elsewhere in this subpart:   (a)Majestic Metals, LLC:  On or about January 22, 2024, and at times prior, the employer did not ensure employees were protected from electrical hazards in that, clearance around disconnect knife switch boxes were less than that specified in Table S-1. Clearance space to the disconnects were impeded by a table and trash bin placed directly in front of the electrical equipment. This condition exposed employees to the hazard of electrical shock and burns.  Abatement Note:  (A)The depth of the working space in the direction of access to live parts may not be less than indicated in Table S-1. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed; (B)The width of working space in front of the electric equipment shall be the width of the equipment or 762 mm (30 in.), whichever is greater. In all cases, the working space shall permit at least a 90-degree opening of equipment doors or hinged panels; and (C)The work space shall be clear and extend from the grade, floor, or platform to the height required by paragraph (g)(1)(vi) of this section. However, other equipment associated with the electrical installation and located above or below the electric equipment may extend not more than 153 mm (6 in.) beyond the front of the electric equipment.
