29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. (B) The periodic inspection shall be conducted to correct any deviations or inadequacies identified. (C) Where lockout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. a) On or before 1/23/2024 the employer did not conduct an adequate periodic inspection of each energy control procedures annually, for equipment such as but not limited to; the energy control procedures for the Paragon Centerless Grinder (model #RC-20B) #1, #2, & #3, to ensure that the documented machine specific procedure was adequate to prevent exposure to rotating parts of the grinders. Nor did the periodic inspections included each authorized employee of that employee's responsibilities to lockout the energy sources such as the electrical disconnect to prevent caught in hazards for each of the rotating wheels located on the grinders.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: (A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or before 1/20/2024, the employer did not provide each machine operator of the Paragon Centerless Grinder (model #RC-20B) lines with adequate authorized employee training, in that employees were not trained to understand how to control energy sources prior to the start of replacing the grinding wheels. Employees did not receive the training necessary to recognize each hazardous energy source including pneumatic, and electric, and how and when to secure each energy source for the grinders prior to grinder wheel changeovers.

29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (1) Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. (2) Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. (3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). (4)Lockout or tagout device application. (i)Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. (ii) Lockout devices, where used, shall be affixed in a manner to that will hold the energy isolating devices in a "safe" or "off" position. (5)(i) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. (ii) If there is a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation shall be continued until the servicing or maintenance is completed, or until the possibility of such accumulation no longer exists. (6) Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. a) On or before 1/20/2024 the employer did not ensure that (d)(1) through (d)(6) were followed by an authorized employee performing servicing and maintenance operations on equipment such as but not limited to; Paragon Centerless Grinder (model #RC-20B) #1, #2, & #3. Employees were exposed to unexpected startup when performing changing of the grinding wheels.

29 CFR 1910.215(a)(2): Guard design. The safety guard shall cover the spindle end, nut, and flange projections. The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard, except: (i) Safety guards on all operations where the work provides a suitable measure of protection to the operator, may be so constructed that the spindle end, nut, and outer flange are exposed; and where the nature of the work is such as to entirely cover the side of the wheel, the side covers of the guard may be omitted. a) On 1/17/2024 the employer did not ensure that the Paragon Centerless Grinder (model #RC-20B), where employees were working, utilized adequate machine guarding method such as but not limited to, suitable measures of protection to the operators. In that employees were exposed to rotating parts of the equipment rotating at 3000 RPMs, as employees reached in to adjust parts. b) On or before 1/17/2024 the employer is requiring employees to reach into the point of operations of the Paragon Centerless Grinder (model #RC-20B) utilizing a wooden stick (12"-18") in order to adjust parts that are coming from the grinding wheels. This exposes employee to caught-in hazards.

29 CFR 1910.215(d)(1): Inspection. Immediately before mounting, all wheels shall be closely inspected and sounded by the user (ring test) to make sure they have not been damaged in transit, storage, or otherwise. The spindle speed of the machine shall be checked before mounting of the wheel to be certain that it does not exceed the maximum operating speed marked on the wheel. Wheels should be tapped gently with a light nonmetallic implement, such as the handle of a screwdriver for light wheels, or a wooden mallet for heavier wheels. If they sound cracked (dead), they shall not be used. This is known as the "Ring Test". a) On or before 1/17/2024 the employer did not ensure that each grinding wheel was sounded (ring tested) prior to the grinding wheels being installed.
