(a) On or about January 24, 2024, at N169 W21010 Meadow Lane, JACKSON, WI 53037, the Adira QH-6025 Brake Press did not have the point of operation adequately guarded, therefore exposing employees to an amputation hazard. (b) On or about January 24, 2024, at N169 W21010 Meadow Lane, JACKSON, WI 53037, the Wysong 1025 Power Shear did not have the point of operation adequately guarded, therefore exposing employees to an amputation hazard.
