Pleasant Ave. Kansas City, MO; were exposed to struck by hazards when powered industrial trucks after undergoing preventative maintenance and inspections were allowed to be placed into service with conditions that would adversely affect the safety of the vehicle. The hazardous condition being tires in poor condition and control lever identification labels missing or illegible. This was observed on the following forklift numbers: F2037,F2988.
