This condition exposed employees to shock and electrocution as well as fire hazards associated with exposed wiring on the electrical box.  (b)UltiMed, Inc. at 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to January 24, 2024, the employer did not ensure that electrical equipment was effectively closed, in that employees were exposed to live electrical circuits associated with a missing electrical breaker in a 240-volt 3 phase power panel on the west wall of the Third Bay area.
