29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 24, 2024, and times prior thereto, in the fabrication shop employees operated a simple pipe bender that did not have one or more methods of machine guarding at the point of operation. This conditions exposed employees to ingoing nip points.

29 CFR 1910.252(b)(2)(iii): Where the work permitted, the welder was not enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide and lamp black, or was enclosed with noncombustible screens similarly painted. Workers or other persons adjacent to the welding areas were not protected from the rays by noncombustible or flameproof screens or shields or were required to wear appropriate goggles: On or about January 24, 2024, at times prior to, employees at this facility were subjected to visible light, infrared, and ultraviolet radiation due to inadequate welding screen enclosures within the welding area.
