29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about January 25, 2024, the employer did not develop nor verify a written Personal Protective Equipment (PPE) hazard assessment for employees adding product into Grit Blaster 1, Grit Blaster 2, and the Plasma Coater to help design the final product. This includes but is not limited to employees using products such Duralum Special White - AB, BP, RF, SK, S2, LAP/F, REF, VLD, brown fused aluminum oxide, and lanthanum Strontium manganite manganese-cobalt dioxide. These products contain the chemical products including aluminum dioxide, titanium dioxide, and other respirable dusts that can present a hazard to employees. These chemicals can cause eye and respiratory irritation through contact with the eyes or when inhaling the product.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  On or about January 25, 2024, the employer did not develop and implement a written training program or employee documentation for employees who conduct grit blasting and plasma coating operations. These employees work in and around hazardous chemical products that can present respirable hazards when loading blasting material into Grit Blaster 1, Grit Blaster 2, and the Plasma Coater. This includes but is not limited to Duralum Special White - AB, BP, RF, SK, S2, LAP/F, REF, VLD, brown fused aluminum oxide, and lanthanum strontium manganite manganese-cobalt dioxide. These products contain the chemical products including aluminum dioxide and titanium dioxide. These chemicals can cause eye and respiratory irritation through contact with the eyes or when inhaling the product.
