On or about 26 October 2023, at the fuel dispensing and storage tanks areas, Varadero at Palmas del Mar, Humacao, PR.  In areas where flammables and combustibles materials were located, it was observed that "no-smoking" signs were missing.  NOTE:	Certification of abatement is not required for this citation item since it was submitted prior to citation issuance. Please disregard the abatement date.

On or about 26 October 2023, at the dry stack yard (patio), Varadero at Palmas, Palmas del Mar, Humacao, PR. Employees were exposed to struck by and crush-by hazards.  A powered industrial truck (Taylor Mo. THM120 Year 2003) missing rated capabilities and charts was driven by employees.    NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

On or about 26 October 2023, at the dry stack yard (patio), Varadero at Palmas, Palmas del Mar, Humacao, PR. Employees were exposed to struck by and crush-by hazards.  The safety belt and horn on the powered industrial truck (Taylor Mo. THM120 Year 2003 ) that was driven by employees were inoperable.    NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).
