29 CFR 1910.180(h)(3)(v): Hoisting, lowering, swinging, or traveling was done while an individual was on the load or hook: Northfield Mount Hermon School, Gill, MA: On or about December 26, 2023, and at times prior, an employee attached his tree climbing saddle and rappelling rope to the crane hook of a truck crane and was lifted to heights greater than 25 feet to perform tree removal work. The employee remained tied to the crane during cutting operation and while the crane was transporting detached sections of tree to the ground.

The section of tree started to swing freely and crushed the employee between the removed section and the existing base of the tree.
