29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) Job site, 2620 Nicholson Road, Sewickley, PA: On or about January 24, 2023, during construction activities, the employer did not ensure that an employee used appropriate fall protection systems/devices when working in an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Job site, 2620 Nicholson Road, Sewickley, PA: On or about January 24, 2023, during construction activities, the employer did not ensure that employees used appropriate fall protection systems/devices on the roof where fall hazards of approximately 18 feet existed in that employees wore harnesses but were not tied off while installing cement board near the edge of the roof. b) Job site, 2620 Nicholson Road, Sewickley, PA: On or about January 24, 2023, during construction activities, the employer did not ensure that employees used appropriate fall protection systems/devices on the roof where fall hazards of approximately 18 feet existed in that the employees relied on red caution tape as fall protection while cutting cement board at the materials cutting area on the roof.
