On January 25, 2024, employees engaged in residential construction activities located at 11168 Portage Dr. Papillion, Nebraska were exposed to potential fall hazards in that the employer had employees using a multi-purpose aluminum ladder that was setup and placed incorrectly as required by this sub-part as the ladder was set on the sloped roof to access the next level without securing the bottom of the ladder to prevent it from slipping out from under the employees during use.
