29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. At the packaging station, right conveyor belt, located at 3258 County Road, 305 South, in Bunnell, FL: On or about January 20, 2024, the employer exposed employees to caught in and amputation hazards, in that the employer did not ensure guarding of the conveyor belt was sufficient prior to permitting employees to work in the conveyor area. b. At the packaging station, left conveyor belt, located at 3258 County Road, 305 South, in Bunnell, FL: On or about January 20, 2024, the employer exposed employees to caught in and amputation hazards, in that the employer did not ensure guarding of the conveyor belt was sufficient prior to permitting employees to work in the conveyor area. c. At the packaging station, side transfer metal conveyor, located at 3258 County Road, 305 South, in Bunnell, FL: On or about January 20, 2024, the employer exposed employees to caught in and amputation hazards, in that the employer did not ensure guarding of the conveyor belt was sufficient prior to permitting employees to work in the conveyor area.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a. At the packaging station, on right side conveyor belt, located at 3258 County Road, 305 South, in Bunnell, FL: On or about January 20, 2024, the employer exposed employees to burns and electrocution hazards, in that live part of electric equipment operating at 50 volts, or more were not guarded against accidental contact while employees were allowed to operate the conveyor belt. b. At the packaging station, on left side conveyor belt, located at 3258 County Road, 305 South, in Bunnell, FL: On or about January 20, 2024, the employer exposed employees to burns and electrocution hazards, in that live part of electric equipment operating at 50 volts, or more were not guarded against accidental contact while employees were allowed to operate the conveyor belt. c. At the packaging station, at the feeding main conveyor belt, located at 3258 County Road, 305 South, in Bunnell, FL: On or about January 20, 2024, the employer exposed employees to burns and electrocution hazards, in that live part of electric equipment operating at 50 volts, or more were not guarded against accidental contact while employees were allowed to operate the cart gate opener switch box.
