29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  At the worksite located at 1710 Walnut Street Kansas City, MO: Employees were exposed to shock and fire hazards in multiple areas of the lab. Laboratory equipment including but not limited to incubators, blenders, PCR prep stations, and fridges were plugged into power taps that were not listed and labeled for the level of power.  a) A PCR prep station plugged into a power tap in the PCR room, as well as another active power tap located on the same table b) A white power tap plugged into the wall of the chemistry room c) Second white power tap plugged into an extension cord in the chemistry room d) A white power tap in the main lab area with an incubator plugged in e) A black power tap in the main lab area f) A second white power tab in the main lab area g) A white power tap by the hoods and washing machine h) A white power tap used in the back storage room

29 CFR 1910.305 (g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  At the worksite located at 1710 Walnut Street Kansas City, MO: Employees were exposed to shock, burn, and or electrocution hazards in the back storage room and other lab rooms. Temporary cords were used in lieu of fixed wiring to power the refrigerator and in the use of power taps throughout the lab.  a) A PCR prep station plugged into a power tap in the PCR room, as well as another active power tap located on the same table b) A white power tap plugged into the wall of the chemistry room c) Second white power tap plugged into an extension cord in the chemistry room d) A white power tap in the main lab area with an incubator plugged in e) A black power tap in the main lab area f) A second white power tab in the main lab area g) A white power tap by the hoods and washing machine h) A white power tap used in the back storage room

29 CFR 1910.1450(e)(1): Where hazardous chemicals were used in the workplace, the employer did not develop and carry out the provisions of a written Chemical Hygiene Plan which was capable of protecting employees from health hazards associated with hazardous chemicals:  At the worksite located at 1710 Walnut Street Kansas City, MO: Employees were exposed to chemical hazards. Employees worked in the lab without a written Chemical Hygiene Plan to protect employees from hazardous chemicals such as, but not limited to, acetonitrile, paraquat, pyridine, and toluene.
