Confined Space: Fuel tank cleaning b. Enclosed Space: Tungsten Inert Gas (TIG)/Metal Inert Gas (MIG) welding  Employees were required to wear 3M half mask tight-fitting respirators with cartridges. The employees were required to wear 3M half mask tight-fitting respirators with 3M 6001 organic vapor cartridges. The employer did not have a sufficient number of respirator models and sizes available to ensure correct fit for all users. This condition potentially exposed employees to fuel vapors which can result in central nervous system depression, mental confusion, and myocardium sensitization which may result in ventricular fibrillation.

The employer did not provide effective training for employees to demonstrate knowledge of the respirator's limitations and capabilities for the confined space environment.  This condition potentially exposed employees to fuel vapors which can result in central nervous system depression, mental confusion, and myocardium sensitization which may result in ventricular fibrillation.  The employer did not ensure that employees were free of facial hair while wearing half mask tight-fitting respirators. This condition potentially exposed employees to central nervous system depression, bronchospasms, asthma, pneumonia, metal fume fever, mental confusion, and myocardium sensitization which may result in ventricular fibrillation.

Building 31C2T: Onboard LCAC 90 and LCAC 62: On and/or before January 25, 2024, employees conducted Tungsten Inert Gas (TIG)/Metal Inert Gas (MIG) welding on aluminum and stainless steel in enclosed spaces with two open sides where other employees worked, staged materials, and frequently walked on/off the vessel.  The employer had not shielded the work from employees working in the vicinity of the welding operations which potentially exposed employees to UV radiation hazards.

Employees wore Black Stallion single layer knit headcovers only and did not consistently wear them correctly as face protection from metal shards, slag, and residual heat. Employees also wore (non fire-retardant) personal clothing with no welding leathers or apron as protection from metal shards molten metals, sparks/flying particulates, and residual heat. The employer had not ensured appropriate face and body protection was properly donned, which potentially exposed employees
