29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) Job site, 7201 McKnight Road, Pittsburgh, PA: On or about January 26, 2024, during construction activities, the employer did not ensure that an employee used appropriate fall protection systems/devices when operating an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled the employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards: a) Job site, 7201 McKnight Road, Pittsburgh, PA: On or about January 26, 2024, during construction activities, the employer did not ensure that appropriate fall protection training was done for an employee who had to work at heights. Deficiencies noted during the inspection included, but were not limited to, failing to ensure that the fall protection system was installed in such a manner that would prevent the employee from hitting the ground below and failing to ensure that there was no equipment below him in the fall zone that he would strike if he fell.
