Employees were not required to wear face protection during machine operation, exposing employees to struck by hazards.

The angular exposure of the grinding wheel periphery and sides for safety guards used on machines known as bench and floor stands exceeded 90 deg. or one-fourth of the periphery: On or about January 26, 2024, where employees performed work on the side of the 30" abrasive wheel mounted on the Besly Model 208 Green Single Spindle Disc Industrial grinder machine, the employer did not ensure that guarding in place only allowed for 90 degrees of angular exposure.
