29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) On or about June 4, 2024, the employer exposed employees to pinch point and struck-by hazards in that the employer failed to ensure employees utilized the machine specific lockout tagout procedure while engaged in replacing the accelerator drive belt on the (AFSM 100) feeder #2. b) 301 Green St, Fayetteville: On or about May 31, 2024, the employer exposed employees to pinch point and struck-by hazards in that the employer failed to ensure employees utilized the machine specific lockout tagout procedure while replacing pulley belts on the Delivery Bar Coded System (DBCS)

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about January 17, 2024, the employer failed to ensure the Loose Mail System (LMS) conveyor belt idler roller and several missing guards throughout the (LMS) were guarded, exposing a supervisor to severe friction burn hazards and employees to amputation hazards.. b) On or about June 20, 2024, the employer failed to ensure the AFSM 100 right-side level change going up was guarded, exposing employees to struck-by hazards.
