29 CFR 1926.760(a)(1):Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. a. On or about January 18, 2024, at a worksite located at 4465 OH-557, Charm, Ohio, the employer failed to ensure employees working on the roof of a steel building at heights over 20 feet were protected from falls. Employees were installing metal decking on the roof of a new build commercial steel building without any form of fall protection in place, exposing the employees to fall hazards over 20 feet.

29 CFR 1926.761(b):Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. a. On or about January 18, 2024, at a worksite located at 4465 OH-557, Charm, Ohio, the employer failed to ensure that temporary employees exposed to fall hazards on a steel erection job site were trained. Temporary employees were installing metal decking on the roof of a new build commercial steel building without being trained on the fall hazards associated with the work being performed, and the methods/equipment to be used to prevent falls which exposed the employees to fall hazards over 20 feet.
