On or about January 18, 2024, at a worksite located at 4465 OH-557, Charm, Ohio, the employer failed to ensure employees working on the roof of a steel building at heights over 20 feet were protected from falls.  Employees were installing metal decking on the roof of a new build commercial steel building without any form of fall protection in place, exposing the employees to fall hazards over 20 feet.

Temporary employees were installing metal decking on the roof of a new build commercial steel building without being trained on the fall hazards associated with the work being performed, and the methods/equipment to be used to prevent falls which exposed the employees to fall hazards over 20 feet.
