Main St. Lakeside-Marblehead OH 43440, did not ensure that LP-Gas propane cylinders used to power industrial heaters within the jobsite stored properly. Six employees were exposed to health and safety fire, explosion, and carbon monoxide respiratory hazards in that two LP-Gas propane cylinders were stored within the third floor of the hotel.

Main St. Lakeside-Marblehead OH 43440, did not ensure that employees were protected from 120 volt electrical shock hazards. Six employees were exposed to 120 volt electrical shocks, in that multiple outlets being used to power drywall mixers, shop vacuums, and electrical tool battery chargers on the third floor of the hotel were not provided with covers or faceplates to prevent electrical contact and shock.

Six employees were exposed to 120 volt electrical shocks, in that multiple flexible cords and cables were used on the third floor of the hotel jobsite in lieu of the fixed wiring of the structure. Multiple flexible cords were being used on the third floor of the hotel jobsite, which were running through exposed ceilings, on walls, and on the floor of the structure.
