29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a) Job site, 225 Clairton Boulevard, Pittsburgh, PA: On or about January 30, 2024, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were used where fall hazards of up to at least approximately 9 1/2 feet existed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) Job site, 225 Clairton Boulevard, Pittsburgh, PA: On or about January 30, 2024, during construction activities, the employer did not ensure that the ladder used to gain access to/egress from the roof was properly set up in that it only extended about 6 inches or so above the landing surface.

29 CFR 1926.20(b)(2): Programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a) Job site, 225 Clairton Boulevard, Pittsburgh, PA: On or about January 30, 2024, during construction activities, the employer did not ensure that a system to identify and correct safety and health issues at the site was implemented. Deficiencies noted during the inspection included, but were not limited to, lack of appropriate fall protection in use where fall hazards of up to at least approximately 9 1/2 feet existed and ladders not being properly extended above the landing area.
