Girard Ave, Philadelphia, PA 19104 - On or about January 30, 2024, the employer did not remove worn or frayed electrical cords that were damaged and in an unsafe condition on the jobsite that posed hazards such as electrical shock or electrocution. Electrical wires cut and electrical taped together with missing outer insulation and were used to power concrete vibrators.
