29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Jobsite, 1540 Oakland Avenue, Indiana, PA 15701: On or about January 26, 2024, electrical equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification. An electrical panel mounted on a wall inside the building was missing a panel cover. b) Jobsite, 1540 Oakland Avenue, Indiana, PA 15701: On or about January 26, 2024, electrical equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification. An electrical outlet hanging to the left of the electrical panel was being used as a pendant style device. The outlet box was equipped with knockouts which could be displaced when handled and the outlet box also lacked proper strain relief to serve as a pendant style device.
