A stairway, ladder, ramp, or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a. Located at 8695 Basil Western Road, Canal Winchester, Ohio, 43110: On or about January 30, 2024, the employer did not ensure that the employees onsite working within a trench located along the southside of the road were provided with an egress method from the trench, such as a ladder.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a. Located along the northside of the road at 8695 Basil Western Road, Canal Winchester, Ohio, 43110: On or about January 30, 2024, the employer did not provide an adequate cave-in protection system for employees working in the approximately 7 foot deep trench performing pipe laying work due to wet and loose soil, undercutting, and vertical wall soil conditions. b. Located along the southside of the road at 8695 Basil Western Road, Canal Winchester, Ohio, 43110: On or about January 30, 2024, the employer did not provide cave-in protection for employees working at the north end wall of the trench between the shielding while performing pipe laying work under the road.
