A) 675 McLean Avenue, Yonkers - NY: An employee was exposed to fall hazards of approximately 40 feet while working in a two-point suspended scaffold. There were two employees in the scaffold and both had harnesses on, however; one of the employees was not tied off, on or about, May 19th, 2023.
