29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Front side of a one-story commercial building: On or about January 26, 2024, and times prior, three employees worked on the roof installing new fa�ade around the exterior of the building. The employees did not use a personal fall arrest system, fall protection or a safety monitoring system exposing them to fall hazards of approximately 16 feet eave to the concrete walkway.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Front side of a one-story commercial building: On or about January 26, 2024, and times prior, employees worked on the roof installing new fa�ade around the exterior of the building. To gain access to the roof, the employees utilized an extension ladder that was not extended 3 feet above the edge of the roof.
