29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  (a) On January 30, 2024, seven employee(s) were observed working to remove roofing on a steep roof and were not protected from a fall hazard of up to approximately thirty (30) feet to the ground below by using either guardrail systems with toe boards, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On January 30, 2024, seven employees using an aluminum extension ladder on the South entrance of the church placed with its side rails approximately one and one-half feet above the roofline were exposed to a fall hazard of approximately twenty-one (21) feet to the concrete walk below while accessing and egressing the roof.
