Location: Worksite On or about January 31, 2024, employees were exposed to fall hazards of over 25 feet, where the employer did not ensure that employees who were required to work from the roof at a height above six feet were trained in the hazards associated with falls and how to avoid them. The employer has previously failed to provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards, as observed by CSHO on January 31, 2024, and for the prior 503(a)(1), or equivalent, serious violations of this standard by this employer: a.)
