29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On January 30, 2024, six employees using an aluminum extension ladder on the South entrance of the church placed with its side rails approximately one and one-half feet above the roofline were exposed to a fall hazard of approximately twenty-one (21) feet to the concrete walk below while accessing and egressing the roof.
