29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about January 31, 2024, the employer failed to ensure employees walking-working surfaces with unprotected sides or edges were adequately protected from falls greater than four feet. Employees utilize adjacent equipment to climb upon to clear overhead jams associated with the 303 presses approximately two times per shift and are exposed to fall heights of approximately 50 inches.  Failure to ensure adequate fall protection from unprotected sides and/or edges exposed employees to potential fractures, sprains, strains, and contusions.

29 CFR  1910.147(c)(7):The employer did not provided training to ensure that the purpose and function of energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are required by employees.  On or before January 31, 2024, the employer did not provide training to ensure that employees followed the correct protocol to clear jammed ends from the overhead conveyer system on press systems 2, 3, and 6.
