29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: (a) Throughout the establishment; On or about January 26, 2024, and dates prior, the employer failed to provide onsite access to trained personnel who could administer first-aid when medical services were not available in near proximity to the worksite. On January 26, 2024, an employee was hospitalized after being struck by a 165-pound workpiece that came off a vertical lathe's material table.

(b) Production area; On or about January 30, 2024, and dates prior, guarding was not provided to protect employees from contacting exposed rotating spindle ends and spindle nuts on pedestal mounted grinding machines equipped with wire wheels.
