At a worksite located at 605 Empire Drive, Jefferson City MO:   Employees were exposed to slip, trip, and fall hazards, struck-by hazards, and fire hazards. This was most recently documented on or about January 23, 2024, and at times prior to, when the employer did not provide walking-working surfaces that were free of accumulated materials including but not limited to shredded paper, wooden pallets, and cardboard boxes.

This was most recently demonstrated on or about January 23, 2024, and at times prior to when the operation of a powered industrial trucks was permitted when the trucks were found to be in an unsafe condition and the employer failed to ensure they trucks were taken out of service until restored to a safe operating condition. The liquified petroleum tank for the powered industrial truck was not fully secured to the equipment.

This was most recently demonstrated on or about January 23, 2024, and at times prior to, when employees would use oxygen and acetylene tanks for welding/cutting purposes. Two oxygen cylinders were being stored next to two acetylene cylinders near the main garage are of the shop.

29 CFR 1910.243(b)(2): Air hose and hose connections used for conducting compressed air to utilization equipment were not designed for the pressure and service to which they were subjected.  At a worksite located at 605 Empire Drive, Jefferson City MO:   The employer failed to protect employees from struck-by hazards. This was most recently demonstrated on or about January 23, 2024, and at times prior to when there was an air hose that was used in part to supply air to pneumatic air lines. The air hose was attached to the hose fitting by a worm drive-type hose clamp which was not designed for use on an air hose. Three clamps were associated with the CT Series Air Compressor and thereby exposing employees to possible whipping action of the hose if the clamp were to come off.

This was last demonstrated on or about January 23, 2024, and at times prior to when flexible cords and or cables (extension cords) were being used by employees to provide power to equipment as a substitute for fixed wiring.  a) A green extension cord was being used to power a Hercules tabletop chop saw. The extension cord was affixed to the structure of the building and being used in lieu of permeant wiring.  b) An orange extension cord with a three-plug extender was being used to power a relocatable power tap and weight scale. The extension cord was affixed to the structure of the building and being used in lieu of permeant wiring.
